WHY THREAD

A changed note should not leave
an old “I’ve read it” looking current.

For an adult family caregiver coordinating a ride or an office contact, THREAD connects a task to the exact wording behind it. This is a research hypothesis illustrated with fictional people, not a finding from patient interviews.

What to watch

Review the first note, acknowledge the transport task, then explicitly link its replacement. Inspect the old acknowledgement and test whether the service rejects an obsolete write.

What is different?

The focus is the visible chain from a source change to a stale handoff. Existing tools also offer sourced education, versioning and coordination; market uniqueness is not proven.

What you can try

Exact quotations, transport assignment, acknowledgements, explicit replacement, recorded history and JSON export. The evidence controls report actual service responses.

What still needs proof
